Information collected, is collected so that we may safely treat you. It is important that we collect this information responsibly, at no time will we divulge your personal information, only persons working directly with you will have access to this information. We will only release your information with a written consent from you or when we are required by law to do so.


All files are stored securely and can only be accessed by healthcare providers. They are kept for up to 10 years after your last visit with us. After this time they can be destroyed appropriately in a way that safeguards your privacy. Paper files are destroyed by shredding, electronic files are destroyed by deleting and hardware when discarded is physically destroyed.


You have a right to ask for and make copies of your records at any time, this request must be made in writing. We will respond to such request within 30 days. We charge a nominal fee for this request.
